The Expert Patients Programme
The Expert Patients Programme is designed to help individuals aged 18 and above who reside, work, or are registered at a GP practice in the boroughs of Merton or Wandsworth. It aims to enhance their health, well-being, and the management of long-term conditions by imparting practical skills and techniques applicable to daily life. The program, offered in either face-to-face or virtual settings, consists of 6 weekly 2.5-hour sessions for in-person courses and 7 weekly 2.5-hour sessions for virtual courses, including an introductory session on Microsoft Teams and general tech guidance. Led by tutors who manage their own long-term health conditions through the techniques taught, the programme covers a range of topics, such as dealing with pain and fatigue, addressing challenging emotions, setting goals, enhancing communication with family, friends, and medical professionals, and accessing other community health services.
